Licensed Baccalaureate Social Worker, LBSW

This is a fully remote position, open to applicants in Texas.

📋 Description

• Provides information and facilitates access to resources and services for patients, families, and/or caregivers, making suitable referrals when necessary.

• Completes patient documentation as required (e.g., patient disability forms, indigent drug program, etc.).

• Serves as an advocate for patients and families in navigating the health care system.

• Communicates identified needs of patients and families to members of the medical team.

• Accurately, appropriately, and timely documents all tasks within the EMR.

• Establishes and nurtures collaborative relationships with community agencies, health care organizations, and other providers.

⛳️ Requirements

• LBSW: Degree in Social Work.

• Licensed to practice in the state of Texas.

• Resides in the state of Texas.

• 0-2 years of social work experience in a healthcare environment, with oncology experience strongly preferred.
