This is a fully remote position, open to applicants in Illinois.
• Define requirements for clinical programs, including workflow intentions and success metrics for intricate care coordination initiatives.
• Collaborate with program leaders in care management, acute escalations, supportive care, and additional programs.
• Formulate requirements and assess both Epic and non-Epic solutions.
• Document workflows and establish measures for program success.
• Collaborate with Informatics and Product from design phase through to go-live and continuous optimization.
• Develop prioritized clinical requirements and use cases for technology roadmaps.
• Convert clinical program logic and workflows into actionable technical specifications.
• Lead the clinical-operations workstream for complex, multi-team launches.
• Validate delivered features through clinical acceptance testing and user acceptance testing (UAT).
• Create scalable frameworks to measure program success.
• Transform plain-language metrics into detailed specifications for technology and data science teams.
• Monitor performance data, identify anomalies, and collaborate with Analytics to address issues.
• Represent Population Health in cross-functional discussions.
• Lead, develop, and retain a high-performing team.
• Prepare executive-level briefings, strategic documents, and presentation materials.
• Identify capability gaps and present risks along with suggested solutions.
• 10–15 years of experience in healthcare program management, population health, or care delivery operations.
• Proven track record of managing complex, multi-stakeholder programs.
• Extensive working knowledge of EHR systems; Epic experience is strongly preferred.
• Capability to translate clinical needs into viable, buildable requirements and evaluate technical trade-offs.
• Direct experience with build/configuration is advantageous but not mandatory.
• Ability to influence technical and build teams as a non-owning collaborator.
• Familiarity with clinical program design logic and technical systems, including Epic, patient segmentation data, and data warehousing concepts.
• Excellent written communication skills with the ability to produce executive-level materials.
• Experience driving clinical-operations workstreams for high-stakes go-lives across matrixed teams.
• Strong analytical skills to identify data issues and determine appropriate escalation paths.
• Experience in people leadership, particularly in developing early-career talent.
• High inclination towards action and comfort in ambiguity.
• Ability to clearly and promptly identify risks to senior leadership.
• Bachelor's degree is required.
