This is a fully remote position, open to applicants in United States.
• Execute various facets of the nursing process while adhering to safe nursing practices.
• Operate in a supervisory capacity as required by Ace Care based on current needs.
• Deliver case management services for medically vulnerable and compromised patients utilizing optimal Case Management methodologies.
• Conduct comprehensive MDS assessments for both new and renewed members.
• Responsible for aiding in the examination of member assessments, implementation, and management of nursing services within the ACE Care framework.
• Initiate and update disease management as necessary.
• Responsible for supporting the initiation, development, and maintenance of the care plan.
• Assist in identifying the needs of families/clients and provide supportive care in alignment with the care plan.
• Deliver educational support to uphold care standards within the nursing and case management departments.
• Maintain consistent communication with the Administrator, other RN and LPN staff, and provide assistance regarding client progress and issues.
• Must be reachable via phone or in person to address medical emergencies.
• Follow up on compliance with physician’s orders and treatments administered by direct care providers.
• Communicate as necessary with the client’s physician, family members, and clients regarding any changes in the care plan or services.
• Assist with the review of the Q.I. program as required.
• Support and participate in provider meetings as needed.
• Provide support and engage in IDT weekly meetings.
• Represent the assessment department during the WAM (Weekly Ace Meeting).
• Assume responsibility for coordinating the physical care of the client or delivering direct care as necessary.
• Maintain knowledge and comprehension of policies and procedures governing the EDWP, formerly known as the SOURCE program.
• Must operate independently in the absence of the administrator to ensure short-term operational continuity.
• Accept additional assignments as deemed appropriate.
• RN Certification (LPN or RN)
• Minimum of two (2) years in a related field
• Proficient in Microsoft 365, with advanced skills in Microsoft Outlook, Word, and Excel
