Intake LPN, Home Health and Hospice

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Oversee the intake process for new patients, which involves accepting referrals, confirming insurance details, and arranging initial assessments.

• Act as the main contact for patients, families, healthcare professionals, and referral sources in relation to Home Health and/or Hospice services.

• Ensure precise and prompt documentation of patient data, including medical history, assessments, care plans, and communications.

• Work collaboratively with interdisciplinary team members to create personalized care plans.

• Organize medical equipment, supplies, therapy services, and community resources.

• Provide education to patients and families regarding medical conditions, treatment options, self-care methods, and support services.

• Track patient progress and treatment responses, reassessing needs and modifying care plans as necessary.

• Manage transitions between care settings, including the discharge from hospitals to Home Health and/or Hospice services.

• Engage in quality assurance activities, audits, reviews, and performance enhancement initiatives.

• Collaborate with physicians, nurses, therapists, and social workers to guarantee coordinated care.

⛳️ Requirements

• Associate's Degree or Certification in Nursing.

• Current Licensed Practical Nurse license issued by the state where services will be provided, or a current multi-state LPN license through the enhanced Nurse Licensure Compact (eNLC).

• A minimum of two years of experience in Home Health and/or Hospice is preferred.

• Two years of experience in utilization review/chart audit is preferred.

• A Bachelor's degree in nursing is preferred.
