Intake LPN, Home Health and Hospice

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee the intake process for new patients, which includes handling referrals, verifying insurance, and scheduling initial assessments.

• Act as the main point of contact for patients, their families, healthcare professionals, and referral sources.

• Guarantee timely and accurate documentation of patient information, medical histories, assessments, care plans, and communications.

• Work collaboratively with interdisciplinary team members to create tailored care plans.

• Organize medical equipment, supplies, therapy services, and community resources.

• Provide education to patients and families regarding conditions, treatment options, self-care techniques, and support services.

• Track patient progress and treatment responses, reassessing needs and modifying care plans as necessary.

• Facilitate transitions between care settings, such as from hospital discharge to Home Health and/or Hospice services.

• Engage in quality assurance activities, audits, reviews, and initiatives for performance improvement.

• Collaborate with physicians, nurses, therapists, and social workers to ensure comprehensive care.

⛳️ Requirements

• Associate's Degree or Certification in Nursing.

• A current Licensed Practical Nurse license from the state where services will be delivered or a current multi-state Licensed Practical Nurse license via the enhanced Nurse Licensure Compact (eNLC).

• Bachelor’s degree in nursing is preferred.

• Two (2) years of experience in Home Health and/or Hospice is preferred.

• Two (2) years of experience in utilization review/chart audit is preferred.

• Weekly scheduled hours: 40.

• This is a non-exempt position.
