Intake Coordinator, Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in South Dakota.

📋 Description

• Receiving transfers from the Patient Enrollment team to perform initial health assessments for newly enrolled patients in our network.

• Addressing patients' inquiries and fostering an open dialogue to comprehend their needs.

• Identifying and coordinating community resources that could enhance patients' care.

• Delivering patient education and promoting health literacy regarding the management of chronic conditions.

• Assisting with medication management, including recognizing potential medication issues, ensuring adherence, and facilitating refills.

• Helping to guarantee the prompt delivery of services to your patients, including Home Health, DME, Home Infusion, and other essential needs.

• This position requires the ability to swiftly adapt to a fast-paced work environment, with most of your shift spent on the phone.

• Active Compact/Multi-State license (LPN) (LVN)

• Technical proficiency in Microsoft Office Suite

• Exceptional written and verbal communication skills
