This is a fully remote position, open to applicants in United States.
• Accountable for processing agency home health referrals promptly and accurately, which includes the precise completion of data entry and client details as indicated in the referral.
• Execute basic diagnosis coding according to referral details.
• Facilitate care coordination for homecare patients while utilizing the Welcome Call Script.
• Capable of assessing referrals from the Clinical Intake team.
• Associate’s Degree is preferred.
• Minimum of one year of experience in Home Health is preferred.
• At least one year of experience in a customer service role.
