This is a fully remote position, open to applicants in North Carolina.
• Respond promptly to inquiries and referrals from designated counties, managing multiple referrals as necessary.
• Gather and evaluate intake, referral, and admission data to determine service suitability and urgency.
• Maintain consistent communication with referral sources to facilitate timely progression of intake.
• Assist with service-related documentation and ensure all intake records (ISP, PCP, psychological assessments, SIS, guardianship, etc.) are complete and accurately uploaded in Dynamics and OnTarget.
• Collaborate with Program Directors to coordinate intake services, encompassing Alternative Family Living and Specialized Consultative Services.
• Identify staffing requirements and communicate effectively with Staffing and Onboarding teams.
• Manage the transition of new clients and intake documentation to the Qualified Professional for clinical handoff.
• Refer clients and their families to Advocacy services and community resources as required.
• Report directly to the Intake Manager and act as the initial point of contact for individuals and families entering services.
• Credentialed or credential-eligible as a Qualified Professional (QP).
• Master's degree in a relevant human services field and at least 1 year of full-time IDD experience, OR a bachelor's degree in a related human services field with a minimum of 2 years of full-time IDD experience, OR a bachelor's degree in an unrelated field with a minimum of 4 years of full-time IDD experience.
• Experience should include providing professional support to individuals with Intellectual and Developmental Disabilities.
• Strong attention to detail and accuracy in documentation and data management.
• Excellent written and verbal communication skills; professional, compassionate, and composed demeanor.
• Highly organized with the capability to manage competing priorities in a fast-paced environment.
• Collaborative team player with excellent interpersonal skills.
• Flexible, adaptable, and proactive in addressing client and organizational needs.
• Ability to establish and maintain therapeutic working relationships with clients, families, and internal teams.
