• Deliver proactive intervention and care coordination for eligible Vaya Health members.
• Ensure these individuals receive the appropriate assessments and services they require.
• Collaborate with the member and care team to address inappropriate levels of care or service gaps through thorough assessment.
• Provide support and may assist with transition planning for state and community hospitals, as well as residential facilities.
• A Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another pertinent human services area.
• A minimum of two (2) years of experience directly working with individuals with Intellectual and Developmental Disabilities (I/DD) or Traumatic Brain Injury (TBI).
• At least two (2) years of previous experience in Long-term Services and Supports and/or Home Community-Based Services coordination, care delivery monitoring, and care management.
• Must fulfill the criteria of being a North Carolina Qualified Professional as defined for the population served in 10A NCAC 27G .0104.
