In-Home Physician

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Conduct in-home health assessments for members of health plans.

• Perform approximately 45-minute visits that involve personalized health discussions.

• Review patients' medications and medical histories.

• Execute physical evaluations.

• Carry out additional diagnostic tests as requested by the health plan, including diabetic eye examinations and spirometry tests.

• Utilize an iPad and clinical workflow for each assessment.

• Refrain from prescribing medications, ordering laboratory tests, or modifying existing treatment plans.

• Establish trusted relationships to enhance health outcomes.

• Travel nationwide to connect individuals with suitable care.

• Travel locally and within authorized states as necessary.

• Conduct virtual visits when required.

• Provide Signify Health services, which encompass diagnostic and preventive services like spirometry, diabetic eye exams, and bone density screenings.

• Perform responsibilities in the private residences of health plan members.

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Multi-state licenses are preferred.

• Must be capable of comfortably and safely performing tasks in homes that may have household pets, secondhand smoke, or other potential airborne allergens or irritants.

• Must be adaptable to travel locally and within authorized states as needed.

• Must be able to conduct virtual visits as required.

• Must meet the qualifications for the specified role.
