In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Conduct In-Home Health Evaluations at the residences of members

• Interact with members, address health-related inquiries, and foster trustworthy relationships

• Assess medications and review medical histories

• Execute physical evaluations

• Administer additional diagnostic tests as prescribed by the member’s health plan, including diabetic eye examinations and spirometry

• Utilize an iPad and clinical workflow for each evaluation

• Travel locally and within the licensed states as required

• Conduct virtual visits as necessary

• Deliver Signify Health services, encompassing diagnostic and preventive measures such as spirometry, diabetic eye exams, and bone density screenings

• Assist in identifying connections to suitable care and bridging gaps in care

⛳️ Requirements

• Nurse Practitioner or Physician Assistant credentials

• Active, unrestricted licenses in the relevant coverage areas

• Board certification is mandatory

• Multi-state licenses are preferred

• Willingness to travel locally and within licensed states as necessary

• Capability to conduct in-home visits in residences that may include household pets, secondhand smoke, and airborne allergens or irritants

• Proficiency in using an iPad and intuitive clinical workflow

• Competence in performing medication and medical history reviews, physical evaluations, and diagnostic and preventive services as required
