In-Home Nurse Practitioner – Per Diem

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Conduct approximately 45-minute In-Home Health Evaluations in the residences of members.

• Connect with members and establish trusted relationships.

• Assess medications and review medical histories.

• Execute physical evaluations.

• Carry out additional diagnostic tests as requested by the member’s health plan, including diabetic eye exams and spirometry.

• Utilize an iPad and clinical workflow for every evaluation.

• Perform virtual visits when necessary.

• Offer Diagnostic & Preventive Services, which encompass spirometry, DEE, and bone density screenings.

• Travel locally and within authorized states as required.

• Assist in identifying connections to suitable care and address gaps in care.

• No obligation to prescribe medications, order lab tests, or modify existing treatment plans.

⛳️ Requirements

• Active, unrestricted licenses in the relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Willingness to travel locally and within licensed states, as necessary.

• Capability to conduct visits in private homes that may feature household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Proficiency in using an iPad and clinical workflow.

• Ability to safely perform in-home clinician responsibilities.
