In-Home Health NP or PA – Full Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct in-home health evaluations lasting approximately 45 minutes

• Interact with health plan members to establish trusted relationships

• Assess medications and review medical histories

• Perform physical evaluations

• Execute additional diagnostic tests as ordered by the health plan, such as diabetic eye exams, spirometry, and bone density screenings

• Utilize an iPad and a user-friendly clinical workflow for each assessment

• Travel locally and within licensed states as required

• Conduct virtual visits as necessary

• Deliver Signify Health services, including Diagnostic & Preventive Services products

• Assist in bridging gaps in care and connecting individuals to suitable healthcare

• Educate members and respond to their health-related inquiries

• Visit individuals nationwide as part of a dedicated network of clinicians

⛳️ Requirements

• Nurse Practitioner or Physician Assistant certification

• Active, unrestricted license(s) in relevant coverage area(s)

• Board certification (mandatory)

• Multi-state licenses (preferred)

• Willingness to travel locally and within licensed states as required

• Capability to conduct visits in private residences, which may include household pets, secondhand smoke, and airborne allergens or irritants

• Proficiency in performing in-home health evaluations utilizing an iPad and clinical workflow

• Availability to work 40 hours per week
