
I/DD Care Manager, Qualified Professional

This is a fully remote position, open to applicants in North Carolina.
• Act as the main point of contact and guide for members with Intellectual and Developmental Disabilities (I/DD) and those dually diagnosed.
• Lead a Team Based Care approach that includes physical health, behavioral health, and long-term supports and services.
• Engage with members in their communities to evaluate current and future needs.
• Develop Person-Centered Care Plans and Individual Support Plans (ISP).
• Organize interdisciplinary team meetings with doctors, specialists, providers, and families.
• Assist members in navigating transitions, such as moving from school to adulthood and returning home from care facilities.
• Support members in achieving maximum independence and identifying paid resources to enhance or maintain health and wellness.
• Inform members and their families of their rights.
• Link members to services and the provider network.
• Bachelor's degree in a human services field such as psychology, social work, nursing, or another relevant human services discipline, along with a minimum of 2 years of full-time experience working with individuals with Intellectual and Developmental Disabilities.
• Alternatively, a bachelor's degree outside the human services field combined with at least 4 years of full-time experience working with individuals with Intellectual and Developmental Disabilities.
• Or, possess a master's degree with a minimum of 1 year of full-time experience working with individuals with Intellectual and Developmental Disabilities.
• At least 2 years of experience with individuals requiring significant Long-Term Services and Supports (LTSS) due to their disability in a community-care environment.
• Must reside in North Carolina or within 40 miles of the NC border.
• Willingness to travel regularly as required to fulfill job responsibilities.
• Knowledge of or eagerness to learn Medicaid regulations, 1915i services, and the Tailored Plan landscape.
• Skills in person-centered planning and care management.
