Housing Outreach Specialist

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Develop and execute the housing strategy and programs for the health plan.

• Assist individuals in obtaining accessible and affordable housing through federal and local initiatives.

• Serve as a liaison between housing agencies and the health plan.

• Collaborate with housing agencies to create and access affordable housing services for members.

• Work alongside Health Plan trainers to create and implement training on affordable housing services in Ohio.

• Operate under the Housing First model while respecting member choice.

• Aid in the development and accessibility of affordable housing services for members receiving LTSS.

• Educate and support Care Coordinators and Support Coordinators in recognizing housing options for members.

⛳️ Requirements

• Candidate must be a resident of Ohio.

• A minimum of 3 years of experience in developing housing and/or housing policy at the local, state, or federal level is required.

• Experience working with special populations, including the elderly, individuals with disabilities, and those facing mental health challenges.

• Proven experience in securing accessible and affordable housing through federal and local programs.

• A combination of education and experience that provides an equivalent background is acceptable.

• Preferred experience with housing, HUD, and homeless populations.

• Familiarity with housing support resources in communities across Ohio is preferred.

• Must reside within commuting distance of an office unless an accommodation is granted.
