This is a fully remote position, open to applicants in Tennessee.
• Collaborate with local hospitals to identify consumers in need of MHC services.
• Determine suitable services based on hospital discharge documentation, face-to-face interactions, and phone meetings.
• Educate and promote MHC services to hospitals in the area.
• Travel to satellite locations to provide educational support to staff as required.
• Build and maintain relationships with hospital staff, discharge planners, and administrative personnel.
• Monitor quality improvement metrics as directed by the Director.
• Track engagement statistics as specified by the Director.
• Compile and present summaries of quality improvement and engagement data to MHC leadership when necessary.
• Analyze ER utilization and inpatient hospitalization reports to identify consumers requiring MHC services.
• Establish connections with local Emergency Rooms to collaborate on the admission of frequent users of emergency services.
• Attend and engage in agency multidisciplinary team meetings as required.
• Act as a representative of the agency for visitors interested in learning about the agency model.
• Ensure attendance at all mandatory in-services, continuing education sessions, and fulfill annual health requirements.
• Adhere to all Corporate Compliance policies, practices, and procedures.
• Report any unethical activities to the supervisor, Corporate Compliance Office, Values Hotline, or Human Resource Director.
• Follow all OSHA and safety policies, practices, and procedures.
• Report unsafe practices or accidents to the supervisor, Safety Supervisor, and/or OSHA Coordinator.
• Comply with relevant regulations regarding the privacy and security of consumer protected health information (PHI) as outlined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
• Engage in proactive team efforts to meet departmental and organizational objectives.
• Perform additional duties as assigned.
• Provide leadership to others through exemplary conduct and sharing of knowledge and skills.
• Adhere to all policies and procedures established by the Agency.
• Master's Degree in a health-related area such as counseling, psychology, or social work.
• Must possess a license or be license eligible (LPC, LMSW, LCSW, LMFT, or equivalent).
• Valid Tennessee Driver’s License is required.
• Acceptable Motor Vehicle Report (MVR) is necessary.
• Must pass an Acceptable Criminal Background Investigation.
