Hospital Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Engage members in the process of care coordination and management.

• Educate individuals on all available services, including natural and community supports, treatment options, and diagnoses.

• Connect members to necessary psychological, behavioral, educational, physical, and dental evaluations.

• Facilitate referrals for health-related services, community-based resources, social support services, and reintegration assistance.

• Assess the effectiveness of the care plan and reintegration supports.

⛳️ Requirements

• Must fulfill Qualified Professional (QP) criteria.

• Fully licensed by the North Carolina governing board that regulates Human Services professions; OR

• Fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience with the MH/DD/SA population.

• A graduate with a master’s degree in a human service field and at least one year of full-time, pre or post-graduate degree experience with the MH/DD/SA population.

• A graduate with a bachelor's degree in a human service field and a minimum of two years of full-time, pre or post-bachelor's degree experience with the MH/DD/SA population.

• A graduate with a bachelor's degree in a field other than human services, having four years of full-time, pre or post-bachelor's degree experience with the MH/DD/SA population.

• Must possess a valid driver’s license.

• Must live within 75 miles of Wayne County, North Carolina.

• Must be willing to travel within the designated catchment area as needed.

• Fully vaccinated against Covid-19, Influenza ( annually between November 1 and March 31), MMR, Varicella, and Tdap (Tetanus, Diphtheria, Pertussis).
