Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Collaborate with on-call teams to ensure coordinated care and resource allocation.

• Precisely document all communications and interventions.

• Maintain a composed and professional attitude while managing urgent calls.

• Offer essential after-hours support for the needs of hospice patients and their families.

• Facilitate timely interventions and care for patients enrolled in hospice services.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR).

• Capability to document notes and information in a fast-paced environment.

• Capacity to manage stress and multitask while receiving calls.

• Excellent communication and critical thinking abilities.

• Ability to work autonomously in a remote setting.

• Regular attendance and proactive communication during all scheduled shifts.

• Dependable internet connectivity during all scheduled shifts.
