Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver telephone triage services for hospice patients and their families

• Evaluate patient conditions and identify suitable next actions

• Work in conjunction with on-call teams to facilitate care and resources

• Thoroughly document all interactions and interventions

• Maintain a composed and professional attitude while managing urgent calls

⛳️ Requirements

• Current multistate Registered Nurse (RN) license

• Experience in hospice, palliative, or end-of-life care is highly preferred

• Proficiency with technology and electronic medical records (EMR) for documenting calls

• Capability to quickly type documentation and notes in a fast-paced setting

• Ability to manage stress and multitask while receiving calls

• Excellent communication and critical thinking skills

• Capacity to work independently in a remote setting

• Reliable attendance, effective communication, and dependable internet access during all scheduled hours
