Hospice Triage RN, PT

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and determine the appropriate next steps.

• Collaborate with on-call teams to effectively coordinate care and resources.

• Accurately document all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

• Provide after-hours support and facilitate timely interventions for hospice patients and families.

⛳️ Requirements

• Must possess an active multistate Registered Nurse (RN) license.

• Previous experience in hospice, palliative, or end-of-life care is highly preferred.

• Comfortable using technology and electronic medical records (EMR).

• Capable of typing documentation and notes in a fast-paced environment.

• Ability to manage stress and multitask while handling calls.

• Strong communication and critical thinking skills are essential.

• Ability to work independently in a remote setting.

• Consistent attendance and active communication during scheduled shifts are required.

• Must have reliable internet connectivity during all scheduled shifts.

• A base pay of $25 per hour, with the potential to increase the hourly rate up to $28 per hour within the first 6 months of employment.

• $2 shift differential for overnight shifts.

• $2 shift differential for weekend shifts (Friday evening, Saturday & Sunday).
