Hospice Triage RN, PT, 3:30p-11p, Rotating Sat & Sun 9:30a-4p CST

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Conduct telephone triage for hospice patients and their families

• Evaluate patient conditions and identify appropriate next steps

• Collaborate with on-call teams to facilitate care and resource allocation

• Ensure accurate documentation of all communications and interventions

• Maintain professionalism and composure while managing urgent calls

• Offer essential after-hours support for the needs of hospice patients and families

• Coordinate prompt interventions and care for patients utilizing hospice services

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license

• Experience in hospice, palliative, or end-of-life care is highly preferred

• Proficiency with technology and electronic medical records (EMR) for call documentation

• Capability to type documentation and notes efficiently in a dynamic environment

• Ability to manage stress and multitask while handling incoming calls

• Excellent communication and critical thinking abilities

• Capacity to work autonomously in a remote setting

• Consistent attendance and proactive communication during scheduled shifts

• Dependable internet access during all assigned shifts
