Hospice After Hours Registered Nurse, RN

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Provide compassionate end-of-life care to patients in their homes.

• Deliver patient-centered hospice care with empathy and respect.

• Collaborate with a multidisciplinary team dedicated to ensuring comfort, dignity, and quality of life.

• Address after-hours and on-call needs of patients.

• Conduct urgent assessments and effectively manage symptoms and pain.

• Offer real-time compassionate support at the end of life.

• Assist and guide patients and their families during evenings, nights, weekends, and during crisis situations.

• Coordinate care requirements using clinical judgment and critical thinking skills.

• Adapt to changing patient conditions promptly.

• Maintain precise, timely, and compliant electronic medical record (EMR) documentation.

• Work autonomously while traveling to patient residences within the designated area.

⛳️ Requirements

• Must possess an active Registered Nurse (RN) license in the state of employment.

• A minimum of two years of RN experience is required.

• Clinical expertise necessary to provide high-quality patient care.

• Prior experience in hospice care is preferred.

• Must have a current driver's license.

• Capability to dedicate approximately 20% of your day to driving to and from patient locations.

• Successful completion of a drug screening prior to employment is mandatory.

• Must be legally authorized to work in the United States.
