This is a fully remote position, open to applicants in Virginia.
• Conduct in-home infusion therapy visits with patients based on the established care plan.
• Consistently prepare clinical and nursing notes, documenting patient interventions in both clinical and non-clinical records.
• Serve as a clinical resource, collaborating closely with other pharmacy departments.
• Stay informed about drug side effects and potential interactions.
• Provide telephone and in-person education to patients regarding therapy protocols, drug side effects, and home infusion procedures.
• Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.
• Act as a liaison during initial and ongoing consultations between the patient, their family, and the pharmacy to facilitate a smooth therapy process.
• Conduct thorough, initial, and periodic head-to-toe assessments for patients, initiating preventative and rehabilitative care as necessary.
• Utilize health assessment data, along with input from the agency team, physician, patient, and family, to identify patient needs.
• Manage expectations of patients and families concerning services, outcomes, and discharge goals.
• Establish primary and secondary diagnoses based on patient assessments and the focus of home health care.
• Create care plans that include skilled interventions, medical supplies, and necessary services to achieve set goals.
• Regularly assess patient progress in collaboration with team members and adjust care plans as needed.
• Perform additional duties as assigned.
• Graduate from an accredited nursing program with a valid RN state license.
• Minimum of 2 years of experience in home infusion and infusion centers.
• At least 2 years of experience with IV Immunoglobulins and IV Biologics.
• CRNI certification (Certified Registered Nurse Infusion) is preferred.
• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.
• Strong interpersonal, time management, and organizational skills.
• Excellent written and verbal communication abilities.
• Organizational skills to maintain consistently accurate records.
• Capability to direct information to the appropriate healthcare provider.
• Proficient in computer skills, including word processing, and adept in using the internet and email efficiently.
• Current CPR and BLS (Red Cross and AHA) certification, along with a valid driver’s license, reliable transportation, and automobile insurance.
