This is a fully remote position, open to applicants in United States.
β’ Perform in-home infusion therapy visits with patients in accordance with the established care plan.
β’ Consistently prepare clinical and nursing documentation, recording patient interventions in both clinical and non-clinical records.
β’ Serve as a clinical resource, collaborating effectively with other pharmacy departments.
β’ Keep updated on drug side effects and potential interactions.
β’ Provide patient education through both telephone and in-person interactions regarding therapy protocols, drug side effects, and home infusion procedures.
β’ Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.
β’ Act as a liaison during initial and ongoing consultations, connecting the patient, their family, and the pharmacy to facilitate a smooth therapy experience.
β’ Conduct thorough initial and periodic head-to-toe assessments for patients, initiating preventive and rehabilitative care as necessary.
β’ Utilize health assessment data along with input from the agency team, physician, patient, and family to ascertain patient needs.
β’ Manage expectations of patients and families concerning services, outcomes, and discharge goals.
β’ Establish primary and secondary diagnoses based on patient assessments and the focus of home health care.
β’ Formulate care plans that include skilled interventions, medical supplies, and necessary services to achieve set goals.
β’ Regularly assess patient progress in collaboration with team members, adjusting care plans as required.
β’ Undertake additional duties as assigned.
β’ Must be a graduate of an accredited nursing program and hold a current RN state license.
β’ At least 2 years of experience in home infusion and infusion center settings.
β’ Minimum of 2 years of experience with IV Immunoglobulins and IV Biologics.
β’ CRNI certification (Certified Registered Nurse Infusion) is preferred.
β’ Preferred experience includes peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.
β’ Strong interpersonal, time management, and organizational skills are essential.
β’ Excellent written and verbal communication abilities.
β’ Organizational skills to maintain consistently accurate records.
β’ Capability to direct information to the appropriate healthcare provider.
β’ Proficient in computer skills, particularly in word processing, and competent in using the internet and email.
β’ Current CPR and BLS (Red Cross and AHA) certification, a valid driverβs license, reliable transportation, and automobile insurance are required.
