This is a fully remote position, open to applicants in South Carolina.
• Perform in-home infusion therapy visits in alignment with the established care plan.
• Prepare clinical and nursing documentation while recording patient interventions in both clinical and non-clinical records.
• Serve as a clinical resource and collaborate effectively with pharmacy departments.
• Stay informed on drug side effects and interactions.
• Provide education on therapy protocols, drug side effects, and home infusion procedures through telephone and in-person interactions.
• Clearly communicate therapy plans, medications, and the importance of adherence to patients.
• Act as a liaison between patients, families, and the pharmacy.
• Conduct thorough initial and ongoing head-to-toe assessments.
• Assess patient needs utilizing data from assessments and input from the care team, physician, patient, and family.
• Manage expectations of patients and families concerning services, outcomes, and discharge goals.
• Identify primary and secondary diagnoses.
• Create care plans that include interventions, necessary supplies, and services required.
• Monitor patient progress and adjust care plans as necessary.
• Carry out other assigned responsibilities.
• Graduate from an accredited nursing program with a current RN state license.
• Minimum of 2 years of experience in home infusion and infusion centers.
• At least 2 years of experience working with IV Immunoglobulins and IV Biologics.
• CRNI certification is preferred.
• Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred.
• Strong interpersonal, time management, and organizational skills are essential.
• Excellent written and verbal communication skills.
• Capability to maintain consistently accurate records.
• Ability to direct information to the appropriate healthcare provider.
• Proficient in computer skills, encompassing word processing and effective use of the internet and email.
• Current CPR and BLS certification from the Red Cross and AHA.
• Valid driver’s license is required.
• Reliable transportation and automobile insurance are necessary.
• Capability to sit or stand, navigate patients’ homes, operate a computer, communicate via telephone and text, and lift up to 20 pounds.
