This is a fully remote position, open to applicants in Rhode Island.
• Conduct in-home infusion therapy visits with patients in alignment with the established care plan.
• Prepare clinical and nursing documentation, consistently recording patient interventions in both clinical and non-clinical records.
• Serve as a clinical resource, collaborating effectively with other pharmacy departments.
• Maintain up-to-date knowledge regarding drug side effects and interactions.
• Provide both telephone and in-person patient education on therapy protocols, drug side effects, and home infusion procedures.
• Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.
• Act as a liaison during initial and ongoing consultations, facilitating communication between the patient, their family, and the pharmacy to ensure a smooth therapy process.
• Conduct comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care.
• Utilize health assessment data and input from the agency team, physician, patient, and family to identify patient needs.
• Manage expectations of patients and families regarding services, outcomes, and discharge goals.
• Establish primary and secondary diagnoses based on patient assessments and home health care focus.
• Create care plans that include skilled interventions, medical supplies, and necessary services to achieve goals.
• Regularly assess patient progress in collaboration with team members and adjust care plans as necessary.
• Execute other duties as assigned.
• Graduate of an accredited nursing program with a current RN state license.
• A minimum of 2 years of experience in home infusion and infusion centers.
• Over 2 years of experience with IV Immunoglobulins and IV Biologics.
• CRNI certification (Certified Registered Nurse Infusion) is preferred.
• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.
• Strong interpersonal, time management, and organizational skills.
• Excellent written and verbal communication abilities.
• Organizational skills to maintain consistently accurate records.
• Capability to direct information to the appropriate healthcare provider.
• Proficient in computer skills, including word processing and efficient use of the internet and email.
• Current CPR and BLS (Red Cross and AHA) certification, valid driver’s license, reliable transportation, and automobile insurance.
