This is a fully remote position, open to applicants in Oklahoma.

📋 Description

• Perform in-home infusion therapy visits with patients in line with the care plan.

• Prepare clinical and nursing documentation while consistently recording patient interventions in both clinical and non-clinical formats.

• Serve as a clinical resource, collaborating effectively with other pharmacy departments.

• Stay informed about drug side effects and potential interactions.

• Provide both telephone and in-person education to patients regarding therapy protocols, drug side effects, and home infusion processes.

• Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.

• Act as a liaison during initial and ongoing consultations between the patient, their family, and the pharmacy, facilitating a smooth therapy process.

• Conduct thorough initial and periodic head-to-toe assessments for patients, initiating preventative and rehabilitative care as necessary.

• Utilize health assessment data alongside input from the agency team, the physician, the patient, and their family to identify patient needs.

• Manage expectations for patients and families regarding services, outcomes, and discharge objectives.

• Establish primary and secondary diagnoses based on patient assessments and the focus of home health care.

• Create care plans that incorporate skilled interventions, medical supplies, and essential services to achieve desired goals.

• Continuously evaluate patient progress in partnership with team members and adjust care plans as required.

• Undertake additional responsibilities as assigned.


⛳️ Requirements

• Graduate from an accredited nursing program with an active RN state license.

• Minimum of 2 years of experience in home infusion and infusion centers.

• At least 2 years of experience with IV Immunoglobulins and IV Biologics.

• CRNI certification (Certified Registered Nurse Infusion) is preferred.

• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.

• Strong interpersonal, time management, and organizational skills.

• Excellent written and verbal communication abilities.

• Capability to maintain consistently accurate records in an organized manner.

• Skill in directing information to the relevant healthcare provider.

• Proficient in computer skills, including word processing, and effective use of the internet and email.

• Current CPR and BLS (Red Cross and AHA) certification, valid driver's license, reliable transportation, and automobile insurance.
