This is a fully remote position, open to applicants in Ohio.

📋 Description

• Conduct in-home infusion therapy visits in accordance with the established plan of care.

• Prepare clinical and nursing documentation, recording patient interventions effectively.

• Serve as a clinical resource while collaborating with pharmacy departments.

• Keep updated on drug side effects and potential interactions.

• Deliver both telephone and in-person education regarding therapy protocols, medication side effects, and home infusion procedures.

• Ensure that patients have a clear understanding of their therapy plans, medications, and adherence requirements.

• Act as a liaison between patients, their families, and the pharmacy.

• Conduct thorough initial and periodic head-to-toe assessments of patients.

• Initiate preventative and rehabilitative care measures.

• Assess patient needs utilizing data from assessments and feedback from the care team, physician, patient, and family members.

• Manage patient and family expectations concerning services, outcomes, and discharge objectives.

• Establish primary and secondary diagnoses based on patient assessments and the focus of home health care.

• Develop comprehensive care plans that include skilled interventions, medical supplies, and necessary services.

• Evaluate patient progress and adjust care plans as necessary.

• Perform additional duties as assigned.


⛳️ Requirements

• Graduate from an accredited nursing program.

• Possess a current RN state license.

• Have 2+ years of experience in home infusion and infusion centers.

• Have 2+ years of experience with IV Immunoglobulins and IV Biologics.

• CRNI certification is preferred.

• Experience with peripheral IV starts, infusion therapy, and phlebotomy for adult and pediatric patients is preferred.

• Exhibit effective interpersonal, time management, and organizational skills.

• Demonstrate strong written and verbal communication skills.

• Ability to maintain consistently accurate records.

• Capability to direct information to the appropriate healthcare provider.

• Proficient in computer skills, including word processing and internet/email usage.

• Hold current CPR and BLS certifications from the Red Cross or AHA.

• Have a valid driver's license.

• Reliable transportation and automobile insurance are required.

• Ability to meet physical demands, including lifting up to 20 pounds occasionally.
