This is a fully remote position, open to applicants in Missouri.
• Conduct in-home infusion therapy visits with patients in accordance with the established care plan.
• Consistently prepare clinical and nursing notes while documenting patient interventions in both clinical and non-clinical records.
• Serve as a clinical resource, collaborating with other pharmacy departments.
• Maintain an understanding of drug side effects and interactions.
• Provide both telephone and in-person patient education regarding therapy protocols, drug side effects, and home infusion procedures.
• Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.
• Act as a liaison during initial and ongoing consultations between patients, their families, and the pharmacy to facilitate a seamless therapy process.
• Conduct thorough initial and periodic head-to-toe assessments for patients, initiating preventative and rehabilitative care as needed.
• Utilize health assessment data and inputs from the agency team, the physician, the patient, and their family to identify patient needs.
• Manage expectations of patients and families regarding services, outcomes, and discharge goals.
• Establish primary and secondary diagnoses based on patient assessment and home health care focus.
• Develop care plans that include skilled interventions, medical supplies, and necessary services to achieve set goals.
• Regularly assess patient progress in collaboration with team members and adjust care plans as necessary.
• Perform additional duties as assigned.
• Graduate from an accredited nursing school with an active RN state license.
• Minimum of 2 years of experience in home infusion and infusion centers.
• Over 2 years of experience with IV Immunoglobulins and IV Biologics.
• CRNI certification (Certified Registered Nurse Infusion) is preferred.
• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.
• Strong interpersonal, time management, and organizational skills are essential.
• Excellent written and verbal communication skills.
• Ability to maintain consistently accurate organizational records.
• Capability to direct information to the appropriate healthcare provider.
• Proficient in computer skills, including word processing, and adept at using the internet and email efficiently.
• Current CPR and BLS (Red Cross and AHA) certification, a valid driver’s license, reliable transportation, and automobile insurance are required.
