This is a fully remote position, open to applicants in Illinois.
• Conduct in-home infusion therapy visits with patients in accordance with the established care plan.
• Consistently prepare clinical and nursing documentation, detailing patient interventions in both clinical and non-clinical records.
• Serve as a clinical resource, collaborating effectively with various pharmacy departments.
• Maintain up-to-date knowledge regarding drug side effects and interactions.
• Provide both telephone and in-person education to patients about therapy protocols, drug side effects, and home infusion processes.
• Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.
• Act as a liaison for patients during initial and ongoing consultations, facilitating communication between the patient, their family, and the pharmacy to guarantee a smooth therapy experience.
• Conduct thorough initial and periodic head-to-toe assessments for patients and initiate preventive and rehabilitative care.
• Utilize health assessment data, along with input from the agency team, physician, patient, and family, to identify patient needs.
• Manage expectations of both patients and families regarding services, outcomes, and discharge objectives.
• Establish primary and secondary diagnoses based on patient assessments and home health care focus.
• Create care plans that incorporate skilled interventions, medical supplies, and necessary services to achieve set goals.
• Regularly assess patient progress in collaboration with team members and adjust care plans as necessary.
• Perform additional duties as assigned.
• Graduate from an accredited nursing program with a valid RN state license.
• A minimum of 2 years of experience in home infusion and infusion centers.
• At least 2 years of experience with IV Immunoglobulins and IV Biologics.
• CRNI certification (Certified Registered Nurse Infusion) is preferred.
• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.
• Strong interpersonal, time management, and organizational skills.
• Excellent written and verbal communication abilities.
• Organizational skills necessary to maintain consistently accurate records.
• Capability to relay information to the appropriate healthcare provider.
• Proficient in computer applications, including word processing, as well as efficient use of the internet and email.
• Current CPR and BLS (Red Cross and AHA) certifications, along with a valid driver’s license, reliable transportation, and automobile insurance.
