This is a fully remote position, open to applicants in Illinois.
β’ Conduct in-home infusion therapy visits with patients in accordance with the established care plan.
β’ Consistently prepare clinical and nursing documentation, detailing patient interventions in both clinical and non-clinical records.
β’ Serve as a clinical resource, collaborating effectively with various pharmacy departments.
β’ Maintain up-to-date knowledge regarding drug side effects and interactions.
β’ Provide both telephone and in-person education to patients about therapy protocols, drug side effects, and home infusion processes.
β’ Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.
β’ Act as a liaison for patients during initial and ongoing consultations, facilitating communication between the patient, their family, and the pharmacy to guarantee a smooth therapy experience.
β’ Conduct thorough initial and periodic head-to-toe assessments for patients and initiate preventive and rehabilitative care.
β’ Utilize health assessment data, along with input from the agency team, physician, patient, and family, to identify patient needs.
β’ Manage expectations of both patients and families regarding services, outcomes, and discharge objectives.
β’ Establish primary and secondary diagnoses based on patient assessments and home health care focus.
β’ Create care plans that incorporate skilled interventions, medical supplies, and necessary services to achieve set goals.
β’ Regularly assess patient progress in collaboration with team members and adjust care plans as necessary.
β’ Perform additional duties as assigned.
β’ Graduate from an accredited nursing program with a valid RN state license.
β’ A minimum of 2 years of experience in home infusion and infusion centers.
β’ At least 2 years of experience with IV Immunoglobulins and IV Biologics.
β’ CRNI certification (Certified Registered Nurse Infusion) is preferred.
β’ Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.
β’ Strong interpersonal, time management, and organizational skills.
β’ Excellent written and verbal communication abilities.
β’ Organizational skills necessary to maintain consistently accurate records.
β’ Capability to relay information to the appropriate healthcare provider.
β’ Proficient in computer applications, including word processing, as well as efficient use of the internet and email.
β’ Current CPR and BLS (Red Cross and AHA) certifications, along with a valid driverβs license, reliable transportation, and automobile insurance.
