Home and Community Support Professional, Family/Friend Caregiver

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide individualized support to a consumer under the Medicaid waiver

• Record notes within 24 hours following the conclusion of a shift

• Complete Annual In-Service Training, which includes topics such as Respect and Dignity, Communication Training, Health Care Coordination, and more

⛳️ Requirements

• Must be a minimum of 18 years old

• Possession of a valid Driver's License - Learner's Permit and State ID are not acceptable

• Current Auto Registration

• Valid Auto Insurance Card

• Driving Record

• Social Security Card, Passport, or another approved form of identity verification

• Recent Tuberculosis Screening (Negative - must be conducted within the last 3 months)

• CPR/First Aid Certification (Front and Back) - Certification must be obtained in person

• County Criminal History Record from each county of residence or employment over the past 3 years

• Background Check for any state where you have lived in the past 3 years

• State (Limited) Criminal History Report - Indiana
