This is a fully remote position, open to applicants in Wisconsin.
• Efficiently upload and categorize external records utilizing relevant software, directing documentation to clinicians as necessary.
• Generate or choose the appropriate patient, encounter, and/or order while assigning the correct document type and description during the indexing process into the EHR.
• Integrate internal and external clinical documentation from both electronic and paper sources through work queues, rounding, eDelivery, or direct importing into the EHR.
• Achieve set benchmarks for quality, accuracy, and productivity.
• Utilize medical terminology and nomenclature to determine documentation requirements based on patient service areas and service levels.
• Review medical records for any missing documentation and signatures.
• Assign and modify medical record deficiencies by the designated provider in the chart management system in accordance with regulations, bylaws, and policies.
• Offer support and education to clinicians regarding the completion of records.
• Collaborate with Data Integrity and other departments on documentation discrepancies and chart corrections.
• Inform leadership for quality reviews and privacy investigations.
• Ensure that scanned documents are clear, legible, accurately positioned, and easily readable by end users.
• Act as the primary contact for record completion support for clinicians and other healthcare providers.
• High School Diploma or equivalent qualification.
• Generally requires 0-1 years of experience in the healthcare field.
• Strong computer and typing skills.
• Capability to learn Epic, OnBase, Solarity, Microsoft Office, legacy archives, Teams, and electronic communication and meeting platforms.
• Exceptional attention to detail and accuracy.
• Ability to prioritize tasks and perform under pressure in a fast-paced setting with time constraints.
• Capability to work independently and make decisions with minimal oversight while upholding quality and productivity standards.
• Ability to collaborate effectively in a diverse team environment.
• Flexibility to adapt to a dynamic environment and shift tasks while maintaining quality and accuracy.
• Openness to receive and implement constructive feedback and take personal responsibility for success.
• Strong interpersonal and communication abilities.
• Capacity to protect confidential health information (PHI).
• Basic understanding of HIPAA regulations.
• No licensure, registration, and/or certification required.
