This is a fully remote position, open to applicants in Canada.
• Aids in enhancing primary care within the family practice environment while building quality improvement capabilities across practice teams through a customized practice facilitation approach.
• Concentrates on refining health technology platforms and workflows (e.g., EMRs, virtual care, clinical decision support tools, etc.) in practice to empower physicians to embrace the principles of the Patient Medical Home and maintain practice enhancements.
• Collaborates closely with key stakeholders (e.g., Divisions of Family Practice, health authorities, Ministry of Health, etc.) in the community to provide practice support services that align with FPSC guidance and the needs of both practice and community.
• A Bachelor's degree in Health Services, Health Information Management, Business Administration, or a related field, along with 3-5 years of experience, or an equivalent combination of education and experience.
• Strong understanding of the BC health system and the operational context of primary care.
• Experience in a family practice or a supporting role is considered an asset.
• Expertise in health technology with the ability to navigate the functionalities: comprehensive understanding of one or two major health technology platforms in BC, knowledge of common workflows, and appropriate processes for different practice types (e.g., solo, group, blended funding).
• Ability to identify data integrity gaps and assist practices in developing clinical data standards for consistent measures and processes.
• Proficient in extracting quality improvement-based measures and related queries.
• Proven understanding and application of quality improvement methodologies, ideally in healthcare settings, including the ability to recognize the connections between workflow and structured data capture, generate meaningful reports, extract and utilize measures, and assess success while motivating providers and teams.
• Successfully employs coaching strategies to inspire individuals and teams to enhance their performance, using inquiry and providing constructive feedback.
• Familiarity with one or more EMRs, particularly in the clinical/operational roles of specific functionalities such as creating registries, scheduling, visit flow, and registry-based recalls.
• Comprehends the BC healthcare system and how it influences the primary care operational model, including financial incentives and patient conditions and journeys that shape processes, team roles, and structures.
• Experience and training in change management principles to facilitate awareness of the human aspects of change for achieving project success; aids others in recognizing individual readiness for change and employs coaching techniques to foster commitment.
• Proficient in facilitating small group sessions; adept at preparing for productive meetings and utilizing methods and questioning skills to enhance group engagement.
• Demonstrated ability to work effectively within and across teams; builds trusting working relationships, encourages the best in others, and is willing to seek assistance when needed.
• A genuine passion for enhancing primary care and assisting physicians in improving their professional practice.
• Exhibits a service-oriented mindset, anticipating, recognizing, and exceeding customer expectations within a defined scope.
• Skilled at navigating diverse personalities and social situations – adjusts communication styles appropriately to match emotional states or cultural environments while understanding the dynamics within a practice team.
• Travel within the region and to its facilities and communities will be necessary, and reliable transportation must be accessible.
