Health Navigator, LPN

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Collaborate with members, their families, healthcare providers, community resources, and other healthcare team members to coordinate services.

• Identify, facilitate, and secure access to necessary healthcare, social services benefits, and community resources.

• Assist members in navigating the journey towards efficient and effective care.

• Actively monitor incoming calls, conduct outgoing calls, and respond promptly to voice mail requests.

• Identify and evaluate members’ medical, behavioral, social, emotional, and financial needs.

• Provide health education to members.

• Foster relationships with members, their families, and caregivers.

⛳️ Requirements

• A minimum of three (3) years of relevant experience in health and wellness promotion.

• Experience in Behavioral Health is advantageous but not mandatory.

• Patient Navigation certification is preferred or should be obtained within one year of employment.

• Active Licensed Practical Nurse license or a degree in a healthcare-related field along with three years of experience directly related to the specified duties and responsibilities.

• Proficient in Microsoft Office products.

• Strong critical thinking and problem-solving abilities.

• Familiarity with navigating all dimensions of medical, behavioral, and social systems.

• Knowledge of current and emerging medical treatment modalities and best practice guidelines.
