This is a fully remote position, open to applicants in Australia.
• Lead and engage in scheduled telehealth case conferences as a part of multidisciplinary Chronic Care Teams.
• Provide medical oversight and clinical guidance for patients with intricate chronic and multi-system conditions.
• Integrate clinical assessments, pathology results, imaging, and allied health findings into cohesive, evidence-based care plans.
• Conduct chronic disease management reviews and care planning, including GP Management Plans (GPCCMPs) and Team Care Arrangements (TCAs).
• Collaborate with allied health professionals, specialists, and care coordinators to enhance patient outcomes.
• Deliver telehealth consultations for patient follow-ups, clinical reviews, and ongoing management.
• Oversee prescribing, referrals, and investigation requests.
• Ensure clinical safety, prioritization, and escalation pathways are recognized and acted upon.
• Conduct approximately 22 patient reviews during an 8-hour structured session.
• Medical degree (MBBS or equivalent).
• Current registration with the Medical Board of Australia (AHPRA).
• Fellowship with RACGP or ACRRM (VR GP), or eligibility for a 19AB exemption in a DPA location.
• Experience in chronic disease management, preventative health, and complex care coordination.
• Proficiency in prescribing, referrals, and investigation management.
• Strong communication and collaboration skills within multidisciplinary teams.
• Suitable home-office environment and reliable internet connection for telehealth delivery.
• Experience with telehealth consultations and digital health platforms is preferred.
• Experience in team-based or multidisciplinary care models is preferred.
• Interest in or experience with veteran health, DVA processes, or the CVC Program is preferred.
• Familiarity with GP Management Plans (GPCCMPs), Team Care Arrangements (TCAs), and chronic disease care planning is preferred.
• Guaranteed hourly rate — $240/hr for the first 3 months, transitioning to 70% of billings thereafter.
