This is a fully remote position, open to applicants in United States.
β’ Conduct ongoing long-term services and support (LTSS) assessments utilizing person-centered thinking methodologies.
β’ Reach out to members via phone or in-person to define goals and establish priorities.
β’ Assess available resources, create care plans, and identify LTSS providers along with community partnerships.
β’ Formulate and adjust service plans while engaging relevant care-team members in the planning process.
β’ Assist members in navigating LTSS along with their environmental and social requirements.
β’ Facilitate access for members and caregivers to long-term services, social, housing, educational, and other resources.
β’ Foster trust and encourage independence by building collaborative relationships with Care Coordinators, members, and caregivers.
β’ Recognize transition opportunities and collaborate with transition coordinators to support member choices.
β’ Manage referrals for non-capitated services and document all member services and natural supports effectively.
β’ Collaborate and consult with Humana-contracted providers regarding the delivery of LTSS.
β’ Engage in interdisciplinary care team meetings.
β’ Connect members with community resources and third-party payers.
β’ Support members in maintaining Medicaid eligibility.
β’ Work closely with Medical Directors, Geriatricians, and Care Coordinators to ensure integrated service delivery and positive outcomes for members.
β’ Meet members in their environments and dedicate time to assessing their needs and obstacles.
β’ Bachelor's degree in a field related to health and human services.
β’ Minimum of 2 years of relevant experience.
β’ Familiarity with various electronic information applications and software programs.
β’ Participation in Humana's tuberculosis (TB) screening program is required.
β’ A strong desire to contribute to an organization dedicated to enhancing consumer experiences.
β’ Valid driver's license for the state of residence.
β’ Personal vehicle liability insurance that meets the minimum limits required by the residing state or by Humana, whichever is higher.
β’ A separate room with a locked door that can be utilized as a private home office.
β’ Residency in Blackford, Delaware, Grant, Henry, Jay, Madison, or Randolph County, Indiana is necessary.
β’ Previous experience with Medicare and Medicaid recipients is preferred.
β’ Experience with electronic case note documentation and various computer applications is preferred.
β’ Background in health promotion, coaching, and wellness is preferred.
β’ Knowledge of community health and social service agencies along with community resources is preferred.
β’ Access to home internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
