This is a fully remote position, open to applicants in New Jersey.
• Serve as a nurse liaison, coordinating care and sharing essential medical information among the Injured Worker, Insured, and Carrier to ensure appropriate treatment, effective rehabilitation, and a successful return to work.
• Adhere to all policies and procedures outlined in the Case Management (CM) plan; maintain accountability for the CM process.
• Accept referrals as directed by the Director of Case Management.
• Conduct initial contacts as per the CM plan; schedule the initial assessment; gather the date for the next physician appointment.
• Confirm assignments with the referral source and clarify any special handling instructions.
• Cultivate professional relationships with clients; treat claimants with respect and dignity.
• After physician appointments, follow up with the Carrier and Insured according to protocol; maintain consistent communication with the Injured Worker, Insured, and Carrier.
• Document data and billing in CaseAnyplace; submit monthly reports in a timely manner (Preliminary, Initial, Progress, Closing).
• Attend physician appointments to obtain diagnosis, prognosis, treatment plan, rehabilitation duration, estimated return to work (modified or regular duty), and MMI/Full Recovery as necessary.
• Refer to a Vocational Counselor for job analyses (modified or regular duty) when appropriate and approved by the Carrier.
• Recommend IME physicians; coordinate and attend IMEs as needed.
• Organize transportation as required.
• Provide translation services as needed (for bilingual nurses).
• Monitor the treatment plan; attend therapy sessions when appropriate; maintain communication with therapists for updates.
• Request the transfer of files to the Vocational Counselor when suitable (LMS, Voc Rehab).
• Assist the Carrier/Insured with Return to Work planning (modified or regular duty).
• Share information with Defense Attorneys as appropriate.
• Foster teamwork among all staff members.
• Optimize accurate and appropriate billable hours to meet the monthly target (8 hrs/day).
• Keep licensure and certifications up to date; complete required annual training punctually.
• Undertake additional professional responsibilities as assigned.
• Maintain accountability for tasks delegated to non-clinical staff.
• Registered Nurse: Must possess a current, unrestricted state license; licensed in every state where field case management is conducted; capable of performing independent assessments within their scope.
• Discipline Eligibility: Must practice in a U.S. state or territory that permits independent assessment within the scope of practice.
• Education: Completion of an accredited nursing program and ongoing continuing education as required.
• Certification: Preferred to have one or more national certifications within four years of hire (e.g., CCM, CRC, CLNC, CRRN).
• Experience: Preferred two years of full-time direct case management experience for injured workers or two years under supervision.
