This is a fully remote position, open to applicants in Illinois.
• Performs a thorough assessment of member eligibility and needs.
• Suggests strategies for resolving cases.
• Recognizes high-risk factors and necessary services.
• Organizes and executes the activities outlined in the care plan.
• Oversees the progress of the care plan.
• Collaborates with case managers, supervisors, Medical Directors, and other health programs.
• Detects and raises concerns regarding the quality of care.
• Employs negotiation skills to obtain suitable options and services.
• Offers coaching, information, and support to empower members.
• Participates in continuous team meetings with colleagues.
• Preferred experience in case management and discharge planning.
• Minimum of 2 years in behavioral health, social services, or a relevant field aligned with the program focus.
• Managed Care experience is a plus.
• Self-assured working from home/independent thinker, utilizing tools to collaborate and connect with teams virtually.
• Willingness to travel within a designated geographic area for in-person case management as directed by Leadership and/or as business needs dictate.
• Strong analytical and problem-solving abilities.
• Proficient communication, organizational, and interpersonal skills.
• Capability to work autonomously.
• Familiarity with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some specialized proprietary applications.
• Effective computer skills, including navigating multiple systems and proficient keyboarding.
