This is a fully remote position, open to applicants in Missouri.

• Deliver direct assistance to patients and their families in addressing social needs.

• Collaborate effectively with community partners and members of the care team to bridge gaps concerning non-medical health drivers.

• Aid in the development and enhancement of community relationships.

• Identify available resources and community support for families or caregivers.

• Offer information, referrals, and non-clinical support to link individuals with medical, psychosocial, and basic needs services.

• Empower families to advocate on their behalf by providing education, guidance, and supportive tools for system navigation.

• Assist in accessing specific programs and support to address psycho-social needs within Imagine Pediatrics and the broader community.

• Advocate for clients and families with community agencies to reduce service gaps and eliminate barriers to access.

• Connect with relevant internal staff and providers to facilitate system navigation.

• Contribute to the creation of an internal community resource library.

• Bachelor’s degree or higher in a healthcare, behavioral health, or social services-related field is preferred.

• Certification as a Community Health Worker is highly desirable.

• Familiarity with navigating social service agencies is required.

• Must possess reliable transportation, a valid Driver's License, and the ability to provide a copy of the current auto insurance certificate.

• Experience with resource management and telephonic services is necessary.

• Bilingual proficiency in Spanish is required.
