Family Practice Nurse Practitioner – North Carolina, South Carolina Licensed

This is a fully remote position, open to applicants in California, +1 more state.

📋 Description

• Conduct virtual wellness appointments directly with patients in their residences.

• Assess the case load and set up necessary technology.

• Collaborate with members of the in-home clinical team (Sprinters).

• Execute health risk assessments, medication reviews, and cognitive evaluations.

• Deliver comprehensive care coordination, including preventive care, medication management, referrals, and documentation of findings.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner License in California.

• Credentialed Medicaid provider for the state of California (Medi-Cal).

• Eagerness to work in an innovative environment that occasionally requires quick problem-solving and creative thinking.

• Proficient with technology and comfortable utilizing tools such as laptops or mobile devices for charting and HIPAA-compliant messaging applications for care coordination.

• Excellent written and verbal communication abilities.

• Capability to work independently as well as part of a team.

• Successful completion of a national background check and valid clinical license verification.
