Family Practice Nurse Practitioner, California Licensed

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct virtual wellness consultations directly with patients in their residences

• Assess case load and set up technology for patient services

• Manage scheduling and collaborate with in-home team members

• Perform health risk assessments, medication evaluations, and cognitive screenings

• Deliver comprehensive care coordination and management

• Engage with patients to foster meaningful relationships

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner

• Active Family Nurse Practitioner License in California

• Credentialed Medicaid provider for California (Medi-Cal)

• Experience in conducting wellness visits

• Strong problem-solving skills

• Proficiency with technology and comfort in using tools such as laptops or mobile devices for documentation and HIPAA-compliant messaging apps

• Excellent written and verbal communication abilities

• Capability to work independently or collaboratively in a team environment

• Successfully pass a national background check and valid clinical license verification
