• Assess your case load and prepare your technological tools.

• Manage your schedule while collaborating with clinical in-home team members.

• Create warm and inviting connections with a diverse array of patients.

• Perform comprehensive health risk assessments, medication reviews, cognitive screenings, and empower patients.

• Collaborate directly with Sprinters to monitor vital signs, organize blood draws, and address specific patient requirements.

• Deliver thorough care coordination and management, including preventive care measures and documentation of findings.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner License in Oklahoma.

• Consistently demonstrates the highest standards of professionalism, integrity, accountability, confidentiality, care, and compassion to provide top-quality health services.

• Willingness to thrive in a dynamic environment that may require last-minute problem solving and innovative thinking.

• Technologically proficient and comfortable utilizing tools such as laptops or mobile devices for documentation and HIPAA-compliant messaging applications for care coordination.

• Strong written and verbal communication abilities.

• Capable of working autonomously or as part of a team.

• Successfully pass a national background check and have a valid clinical license search.
