Family Nurse Practitioner, Michigan Licensed

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Start your day by reviewing your case load and preparing your technology, ensuring you have all the necessary tools to effectively serve your patients.

• Utilizing user-friendly and modern technology, you will manage your daily schedule and collaborate with our clinical in-home team members (Sprinters) who will visit each patient’s residence.

• Establish a warm and welcoming rapport with a diverse array of patients as you prepare to gather pertinent information and perform various services.

• Execute comprehensive health risk assessments, medication reviews, cognitive screenings, and empower patients with educational insights regarding their health and well-being.

• Collaborate directly with Sprinters to assess vital signs, schedule blood draws, and perform necessary tasks to meet the specific needs of the patient.

• Deliver thorough care coordination and management, including preventive care initiatives, medication management, referrals to specialists, community resources, and documentation of findings.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Possess an active Family Nurse Practitioner license in Michigan.

• Consistently demonstrate the highest levels of professionalism, integrity, accountability, confidentiality, care, and compassion to deliver high-quality health services.

• Willingness to operate in a groundbreaking environment that may require last-minute problem solving and innovative thinking.

• Proficient with technology and comfortable using tools such as laptops or mobile devices for charting and HIPAA-compliant messaging applications for care coordination.

• Strong written and verbal communication abilities.

• Capable of working independently or as part of a team.

• Successfully pass a national background check and validate clinical license search.
