📋 Description

• Perform virtual wellness consultations directly with patients in the comfort of their own homes.

• Utilize medical knowledge to provide a comprehensive array of healthcare services.

• Establish warm and welcoming connections with a diverse patient demographic.

• Engage in proactive care through health risk assessments and educational informational sessions.

• Deliver thorough care coordination and management, including preventive care measures and medication oversight.

⛳️ Requirements

• Must be Board Certified as a Family Nurse Practitioner.

• Hold an active Family Nurse Practitioner license in one of the following states: KY, MI, OH, IL.

• Consistently demonstrate the highest standards of professionalism, integrity, accountability, confidentiality, care, and compassion to deliver high-quality health services.

• Willing to work in an innovative environment that may require quick problem solving and creative thinking.

• Comfortable with technology and adept at using tools such as laptops or mobile devices for documentation and HIPAA-compliant messaging apps for care coordination.

• Excellent written and verbal communication skills.

• Capable of working independently as well as collaboratively within a team.

• Must pass a national background check and maintain a valid clinical license.
