This is a fully remote position, open to applicants in Arizona, +3 more states.
• Perform comprehensive assessments of patients suffering from hypertension, hyperlipidemia, coronary artery disease, diabetes, and various chronic health issues.
• Conduct virtual consultations, which are anticipated to account for 40–60% of clinical responsibilities.
• Develop diagnoses and personalized treatment strategies, incorporating medication management, lifestyle changes, and monitoring suggestions.
• Oversee a prescription refill inbox and queue, adhering to established protocols and guidelines.
• Engage in centralized results management, which includes the review, triage, and follow-up on laboratory and diagnostic findings.
• Utilize digital live portal chat and AI-enhanced tools for prescribing and inbox triage to optimize medication renewals, results management, and patient communication.
• Recognize risk factors, comorbidities, and contraindications to treatment.
• Advise patients on medication adherence and possible side effects.
• Collaborate with primary care physicians, specialists, pharmacists, clinical nursing staff, social workers, family members, and caregivers.
• Employ the company's electronic health information system during visits and provide feedback on the interface.
• Keep precise and up-to-date patient medical records.
• Engage in quality improvement initiatives and pursue ongoing professional development.
• Adhere to relevant laws, regulations, patient privacy standards, and telehealth guidelines.
• Advanced practice provider, either as a Nurse Practitioner or Physician Assistant.
• A minimum of three years of experience in outpatient clinical practice.
• At least two years focused on primary care or a related subspecialty, particularly with an emphasis on hypertension and lipid management.
• Preferably experienced in managing hypertension, hyperlipidemia, and both primary and secondary prevention of atherosclerotic cardiovascular disease.
• Nurse Practitioners must possess active, in-good-standing RN and APRN licenses.
• Physician Assistants must maintain an active, in-good-standing Physician Assistant license.
• All role-related licensure and certification must be in good standing.
• Capability to obtain licensure in designated states within 90 days of hiring, along with additional states as required.
• Comprehensive understanding of cardiometabolic condition management, treatment options, medication oversight, and lifestyle alterations.
• Proficient in utilizing digital live portal chat and emerging AI-driven clinical tools.
• Experience or a strong interest in centralized results management, digital patient engagement, and AI-enhanced clinical processes.
• Preference for candidates with virtual care experience.
• Proficiency in both English and Spanish is preferred.
• Willingness and ability to work Monday through Friday, 8-hour shifts, typically from 8 am to 5 pm MST/PST, with consideration for EST/CST hours.
• Familiarity with STARS/HEDIS measures, clinical comorbidity coding, and identifying gaps in clinical care.
