This is a fully remote position, open to applicants in New York.
• Assist families with children experiencing developmental delays in navigating the Early Intervention (EI) system.
• Coordinate services for children across various agencies.
• Provide information regarding available services, resources, and options.
• Offer case management for families by planning, coordinating, participating in, and monitoring the Individualized Family Service Plan (IFSP).
• Communicate with families to assess their concerns, priorities, resources, strengths, and needs.
• Ensure that services are delivered in accordance with the Individual Family Service Plan (IFSP).
• Aid families in accessing community resources and services.
• Document notes following each contact made.
• Submit required paperwork promptly.
• Communicate with the child’s EI team, which includes providers, outside agencies, municipality, and ACP-EI staff.
• Prior experience in Early Intervention Service Coordination is mandatory.
• Knowledge of child development is advantageous.
• Strong written and verbal communication skills are essential.
• Must be well-organized with effective time management abilities.
• Adherence to agency and municipality policies and procedures is required.
• Ability to work independently while also being an effective team player.
• Availability for daytime meetings is necessary.
• Proficiency in Spanish is required.
