This is a fully remote position, open to applicants in Texas.
• Provide leadership and clinical oversight for the orthopedic home health program.
• Collaborate with physicians, physical therapists, occupational therapists, nurses, and care coordinators to optimize orthopedic recovery plans.
• Develop and implement standardized protocols and pathways for common orthopedic conditions, including total knee/hip replacement, fractures, and post-surgical care.
• Monitor program performance through data analysis, quality metrics, patient outcomes, and satisfaction scores.
• Ensure documentation and care practices meet CMS regulations, payer guidelines, and accreditation standards.
• Conduct training, mentoring, and performance evaluations for orthopedic team members and support staff.
• Coordinate with referral sources, hospital discharge planners, and orthopedic surgeons to facilitate transitions of care to home.
• Participate in case conferences, audits, and interdisciplinary planning.
• Identify and implement opportunities for innovation, growth, and continuous improvement in the orthopedic service line.
• Stay up-to-date on advancements in orthopedic rehabilitation, telehealth, and home-based care models.
• Must be a licensed Physical Therapist (PT), Occupational Therapist (OT), Nurse Practitioner (NP), or Registered Nurse (RN) with orthopedic specialization.
• Minimum of 5 years of clinical orthopedic experience.
• At least 2 years in a leadership or management role.
• In-depth knowledge of home health regulations, clinical documentation standards, and orthopedic recovery pathways.
• Demonstrated ability to lead interdisciplinary teams and manage complex patient populations.
• Excellent organizational, communication, and leadership skills.
• Experience in a home health or post-acute care setting is preferred.
• Advanced certification in orthopedics, such as OCS or ONC, is preferred.
• Familiarity with EMR systems such as Homecare Homebase, WellSky, or Kinnser is preferred.
• Master's degree in healthcare administration, physical therapy, nursing, or a related field is preferred.
• Knowledge of Medicare, PDGM, and OASIS.
