Director, CHW Operations

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee all operations of Community Health Workers across various markets, focusing on workforce planning, deployment, productivity, and performance management.

• Set uniform standards for home visits, community-based interventions, member engagement, outreach efforts, and field documentation.

• Develop management systems that promote coaching, accountability, quality, and professional advancement within the CHW organization.

• Collaborate closely with Nurse Practitioners, RN Care Managers, Behavioral Health Case Managers, and Care Coordinators to ensure smooth integration within care teams.

• Track operational performance and seek ways to enhance member engagement, retention, and outcomes.

• Assist in expanding into new markets while ensuring consistent service delivery and operational excellence.

• Act as the primary leader accountable for the effectiveness and scalability of the CHW function.

⛳️ Requirements

• A minimum of 8 years of leadership experience in healthcare operations.

• Proven experience managing Community Health Workers, care coordinators, patient navigators, outreach teams, or other community-based healthcare workforces.

• Experience in building and managing distributed field teams.

• In-depth knowledge of Medicaid populations and the social determinants of health.

• Proven track record in developing frontline leaders and enhancing operational performance.

• Exceptional communication, coaching, and change management abilities.
