This is a fully remote position, open to applicants in North Carolina.
• Directly oversee and provide supervision for a localized Integrated Care Team that supports Innovations members within a specific region.
• Offer guidance, education, and assistance to staff as necessary.
• Develop, execute, and monitor standards, procedures, and processes with the approval of the Operations Director and Senior Director of IDD Care Management.
• Provide leadership for ongoing programs or projects as assigned.
• Conduct research, needs assessments, or participate in training programs to identify opportunities for Trillium, and assist with their implementation.
• Review dashboard metrics to ensure adherence to Medicaid, HCBS, and Innovations Waiver requirements.
• Monitor critical incidents and address escalated concerns.
• Resolve issues raised by members and providers.
• Oversee an integrated team that delivers localized support in a managed care setting.
• Ensure the application of industry-standard, evidence-based best practices and a whole-person care approach.
• A bachelor's degree in a health or human services discipline.
• A minimum of three (3) years of progressive experience in care management, case management, or care coordination involving MH/SU/DD populations.
• Experience in program development and/or program management is essential.
• A valid driver's license is required.
• Candidates must reside within North Carolina.
• Ability to travel within the catchment area as needed.
• Supervisory experience of a team consisting of two or more employees is preferred.
• Familiarity with Innovations in mental health, substance use, intellectual/developmental disabilities, and/or TBI populations is preferred.
• Experience with Innovations Waiver, HCBS, Care Management, and Managed Care is preferred.
• Passing a drug test is a condition of employment.
