Diabetes Educator – Certified Diabetes Care and Education Specialist

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Assess members for eligibility and suitability for the Diabetes Clinic.

• Conduct thorough evaluations and maintain ongoing engagement with members in the Diabetes Clinic to provide continuous education on all aspects of diabetes management (including disease understanding, medication usage, glucose monitoring, lifestyle counseling, and contingency planning).

• Arrange follow-up appointments for both yourself and our healthcare providers.

• Link members and providers to appropriate resources within Devoted to address clinical concerns.

• Collaborate with members to formulate and achieve shared SMART goals.

• Employ motivational interviewing techniques aimed at fostering positive and lasting behavioral changes.

• Recognize care gaps and offer relevant resources.

• Work alongside other team members in the Diabetes Clinic (such as nurse practitioners, social workers, and pharmacists) to create care plans and enhance medication regimens to assist members in reaching their objectives.

• Keep up-to-date documentation regarding interactions with members and providers.

• Enhance your skill set through participation in educational series both within the Diabetes Clinic and with external continuing education providers.

⛳️ Requirements

• A current unrestricted Compact Registered Nurse license or an unrestricted Registered Dietitian license.

• An active Certified Diabetes Care and Education Specialist (CDCES) credential.

• Minimum of 2 years of experience in clinical diabetes management.

• Experience in remote diabetes management is highly preferred.

• Familiarity with diabetes technology (such as CGMs and insulin pumps).
